Why does such a tragic incident continue to gnaw at our safety? Why are the authorities who have been entrusted wit the responsibility of safeguarding the public failed yet again? Why are we so vulnerable? These are legitimate questions that call for answers.
Allow me to share my thoughts on how a tragedy like this can be prevented. It is imperative that everyone looks out for dangers that lurk as we commute as well as at our workplace. Identifying and highlighting hazards that pose risks goes a long way in mitigating the risks of injury or death. This can come about if we begin to focus on events or situations we observe that do not result in illness, injury or damage but potentially is capable of doing so. The public as well as employees at work should report near misses so that the relevant authorities can take necessary action before it is too late.
